As healthcare professionals we are required by law to gather specific information about your personal details, medical history, non-prescribed and prescribed medication as podiatry is a medical treatment. We ask if you could be open and honest with us when sharing your information as by law we need to keep a record of your treatments, personal and medical data. We gather this information to understand your medical history and how this affects your foot related complaint and we will use this information when we are assessing, diagnosing and formulating your Podiatry Treatment Plan. In order for us to apply our clinical knowledge, we first need to gather the information.
New patient attending clinic
Attending your first appointment requires a little extra time for the podiatrist to carry out a full assessment. You will be required to complete a Personal Information form, which includes a medical questionnaire, and a consent to treat form. You can either download the forms and email them to us at info@ochilspodiatry.co.uk prior to your appointment or you can print them off below and bring them in with you. If you do not have access to a printer you can complete the forms when you arrive.
New patients requesting home visits
The podiatrist will bring all the documentation required to be completed by the patient. If the patient has a Welfare Guardian, please see our Vulnerable Adults section below. Please inform the Podiatrist of any special instructions required before attending the patient’s home.
What do I need to bring for my first appointment?
As healthcare professionals we need to gather certain information about your health and wellbeing, so we need you to be open with us when sharing your information. It is really important that you inform the podiatrist of any previous and current health related issues, what medication you are on and what allergies you may suffer from as this will determine what type of treatment you receive. If applicable, please bring a list of your prescribed medication and any other documentation, such as X-Ray or MRI results you have recently had taken and that are related to your foot complaint.
Why do we need this information?
Podiatrists are regulated by the Health and Care Professions Council (HCPC) and we are bound to follow their clinical standards and the practical guidelines as well as those set out by The College of Podiatry. Therefore, it is important that you inform the podiatrist of any changes to your health and medication so we can continue to provide the right treatment for you. There may be instances where the podiatrist may need to share your personal information and treatment plan with your GP or other healthcare professionals regarding your foot care. Should we need to discuss or refer your medical care, we will need your consent.
